CLINICAL CARE SERVICES WELCOMES NEW NURSE PRACTITIONER, ISABELLE HUBERT

The MCA Department of Health’s Clinical Care Services (CCS) would like to extend a warm welcome to its new Nurse Practitioner, Isabelle Hubert. Isabelle will be seeing clients in the Akwesasne Medical Clinic (AMC) and the Tsi Snaihne Medical Clinic on Fridays providing elder care clinics and offer assistance with end-of-life care. She will be providing same-day and booked appointments, and will begin seeing clients in February 2026. Appointments will be booked through the Akwesasne Medical Clinic.

Isabelle has been a Nurse Practitioner for over 15 years, having graduated from McGill University in Primary Care Nurse Practitioner. She has worked exclusively for the past 10 years in geriatrics (health and care of the elderly), which she feels working with elders is a passion and a privilege.

She has a wide range of medical setting experience, including Suroit Hospital, long-term care facilities, home care and palliative care. She has also been a consultant for the McGill Nurse Practitioner Admission Program and supervised NP students in the geriatric setting.

Isabelle is also certified in the following areas: Canadian Association of MAID Assessors and Providers (CAMAP) and LEAP-Core, which is an inter professional facilitated learning program which focuses on the essential competencies to provide a palliative care approach.

Her work and experience have led to nominations for the Individual Inspiration Award by CISSSMO in 2024 and for the AIPSQ (Quebec Nurse Practitioner Association) Distinction Award in 2025.
