Who can book an appointment at the Elders Clinic?

💬 Community members 60 years of age and older are eligible to book an appointment at the Elders Clinic.

Appointments may include discussion and assessment related to:

• Fall risk and safety
• Cognitive health
• Medication management (polypharmacy)

Where are the Elders Clinics held?

💬 Elders Clinics rotate between the Akwesasne Medical Clinic and the Tsi Snaihne Medical Clinic. Home consultations are also available when needed. 

What can I expect during my appointment?

💬 During your visit, you can expect:

• A review of your current health conditions and management plan.
• A physical assessment (exam).

Do I need a referral to book an Elders Clinic appointment?

💬 No referral is required. Community members can book directly for the Elders Clinic. Isabelle will also see patients who are referred by other healthcare professionals.

Are home visits available for Elders Clinics?

💬 Yes. Home visits are available for Elders who experience mobility or transportation challenges. Transportation support may also be available through the NIHB Medical Transportation Program.
