Continuing Care Services

Continuing Care Services is dedicated to providing comprehensive and compassionate care for the elders of the Akwesasne Mohawk community. Recognizing the wisdom and experience our elders bring, we are committed to ensuring they receive the support, dignity, and respect they deserve. 

Sector 5 Services

Elder Services offers personalized care and assistance for Akwesasne elders aged 60 and over, helping them live safely and independently at home. The program combines practical support, health monitoring, and social connection to enhance quality of life.

Key Programs & Services:

Meal Delivery Program (Meals on Wheels)
Hot, nutritious lunchtime meals are delivered to homebound elders, helping them remain healthy and independent. Beyond the meal itself, clients benefit from daily contact and safety checks from delivery drivers, along with general health information and updates on community events.

Clients must meet at least one of the following criteria:

  • Be homebound.
  • Have impaired mobility or be incapacitated due to accident, illness, or frailty.
  • Lack family or neighbor support.
  • Be unable to prepare meals due to lack of facilities or skills.
  • Be unable to shop or cook for themselves.
  • Require temporary support after hospitalization or illness.

Assessments are completed by the Program Manager/Care Coordinator, with follow-ups and reassessments at least every twelve months for case-managed participants.

MEALS ON WHEELS APPLICATION


Minor Home Maintenance

Helps elders keep their homes safe and functional. Services include light repairs such as tightening loose railings, cleaning wood-burning stoves, replacing smoke detector batteries, and addressing small drain or drywall issues. If the repair is beyond program scope, referrals to trusted contractors are provided.

Care Coordination
Assists elders in managing their health, safety, and social needs, helping them maintain independence and access appropriate resources. 

ELDER LAWN CARE APPLICATION

 

No referral required — clients may self-refer. Providers or caregivers may also make referrals.

Iakhihsohtha Lodge provides short-term, long-term, and respite care for adults and elders, along with a senior day program and congregate dining. Open since 1990, the Lodge fosters dignity, comfort, and a sense of community while supporting independence.

Key Programs & Services:

Short-Term, Long-Term, and Respite Care

  • 20 private rooms and 5 semi-private rooms, each with an attached washroom.
  • Two fully accessible bathing rooms with therapeutic tubs and showers.
  • In-house beauty & barber salon, walking paths, outdoor patios, and garden beds.
  • Handicap-accessible bus for weekly outings including shopping trips, group picnics, and scenic drives.

Ratinatare (Our Visitors) Senior Day Program
Held twice weekly from 9:00 a.m. to 3:00 p.m., the program provides meals, exercise, games, and activities in a supportive group setting. It’s ideal for elders seeking social interaction and for caregivers of individuals with Alzheimer’s or dementia who need a short break.

Congregate Dining
A welcoming space for elders to enjoy meals, games, and social interaction in a lively group setting.

 

How to Apply:

Applications are available at the Lodge. Once completed, a medical assessment will be scheduled to ensure services are suited to the applicant’s needs.

Tsiionkwanonhso:te, meaning "Our Home," provides 24/7 nursing care, medical services, and life enrichment programs in a safe, culturally sensitive, home-like setting. The facility serves adults and elders who require ongoing medical care and daily living assistance.

Key Programs & Services:

Nursing & Medical Services

  • Physicians, registered nurses (RNs), registered practical nurses (RPNs), and personal support workers (PSWs) available around the clock.
  • Restorative care aides, mental health support, and pharmacy services.
  • Access to massage therapy, physiotherapy, occupational therapy, and art therapy.
  • Optional fee-based services: hairstylist, esthetician, private satellite TV, phone service, and in-room climate control.

Leisure Programs
Regular activities take place in lounges, outdoors, and on outings via the accessible bus. Programs include community events, volunteer visits, cultural activities, and seasonal celebrations.

Food Services
Nutritious meals and snacks are prepared under the guidance of a Registered Dietitian. Monthly special meals and food-centered events are part of the calendar.

 

How to Apply:

Applicants must be a community member of Akwesasne with:

  • Canadian Band Number
  • QHIP/OHIP health card
  • Birth certificate
  • Power of Attorney (if applicable)

Admission can also occur through Cornwall Community Access Centre (CCAC). Fees are set according to the Ontario Ministry of Health and Long-Term Care rates.
