Elder Services offers personalized care and assistance for Akwesasne elders aged 60 and over, helping them live safely and independently at home. The program combines practical support, health monitoring, and social connection to enhance quality of life.
Key Programs & Services:
Meal Delivery Program (Meals on Wheels)
Hot, nutritious lunchtime meals are delivered to homebound elders, helping them remain healthy and independent. Beyond the meal itself, clients benefit from daily contact and safety checks from delivery drivers, along with general health information and updates on community events.
Clients must meet at least one of the following criteria:
- Be homebound.
- Have impaired mobility or be incapacitated due to accident, illness, or frailty.
- Lack family or neighbor support.
- Be unable to prepare meals due to lack of facilities or skills.
- Be unable to shop or cook for themselves.
- Require temporary support after hospitalization or illness.
Assessments are completed by the Program Manager/Care Coordinator, with follow-ups and reassessments at least every twelve months for case-managed participants.
