COVID-19 Outbreak Declared

At Tsiionkwanonhso:te (Our Home), we have identified one confirmed positive case of COVID-19 among residents. This positive test result puts Our Home on outbreak status as of Saturday, June 25, 2022.

What happens next:

To reduce the spread and impact of COVID-19, we continue to follow guidance from the Ministry of Long-Term Care.

We are staying in close communication with local health officials (EOHU) to ensure we are taking the appropriate steps under the current circumstances, including:

  • enhanced infection control precautions, such as wearing personal protective equipment (PPE) when we care for residents, increasing the availability of hand sanitizer, and more frequent cleaning and disinfecting of high-touch surfaces. Residents may also be moved in Our Home to prevent the spread of the virus causing COVID-19.
  • screening residents and staff for COVID-19 symptoms, as well as anyone else who must enter our facility. Only staff and designated caregivers who must enter the building will be permitted entry.
  • testing staff and residents for the virus causing COVID-19 based on current protocols and availability of tests.
  • providing and requiring medical grade face coverings for everyone to wear.
  • postponing group activities and helping residents to practice social distancing.

re: Tsiionkwanonhso:te’s Telephones Not Working causing Family Members Concern

“We submitted the PO to BrantTel to purchase 30 additional licenses successfully last Friday, June 17th. We received confirmation from BrantTel, as well. It was supposed to be applied this week and unfortunately it hasn’t.

Our department does understand that our health care facilities require services at all times. Among other emergency services. This, sadly, is out of our capacity.

We are researching alternatives in the near future to avoid circumstances such as this.”

Telephone Issues Reported

June 13, 2022.

The following issues with business telephones (located in five areas) were reported:

  • no dial tone
  • phone will ring once then go to voice mail
  • trying to reset by unplugging the telephone wires for five minutes, then plugging wires back in with no positive result
  • poor voice call quality with phones that worked

Our History

The Mohawk Council of Akwesasne and the Department of Health is pleased to present Tsiionkwanonhso:te, meaning “Our Home”.  Tsiionkwanonhso:te is a 50 bed long term facility that has been serving our Akwesasne Mohawk Territory and surrounding communities since 1994.  Tsiionkwanonhso:te offers many different  services and programs to accommodate each residents needs. 

Our Mission Statement

Contributing to improving the quality of life of adults, Elders and families by delivering programs and services to sustain their health, social, cultural and life enrichment needs.  We deliver services in a warm and caring, fun and safe, home like environment.  We are committed to working cooperatively and professionally to fulfill the diverse needs of our Mohawk People, our Partners and Regulatory Bodies.

Nursing and Medical Services

Our professional providers are fully certified and qualified to offer health care to our residents. (physician, RN, RPN, PSW 24/7, restorative care aide, mental health, pharmaceutical).  We offer massage therapy, physiotherapy, and occupational therapy for residents.  A licensed Esthetician, hairstylist, satellite TV subscription, phone/internet subscription our available as an ‘out of pocket’ expense.

Recreation and Leisure Programs

Tsiionkwanonhso:te staff offers a variety of Activity programs to meet the cognitive, spiritual, emotional, physical, and social needs of the current Resident population. Residents and their caregivers are welcome to participate in any or all of the scheduled activities. Activity Programs are held in the South or West Wing common areas and/or outdoors within a secure area overlooking the mighty St.Lawrence River. Note: COVID-19 precautions affect group activities and the number of participants.

Examples of activities include: Holy Mass services are offered on the second Thursday in the activity room. Bingo games, toss games, Trivia, Puzzles, Special Events to celebrate anniversaries (August 17th opening) and holidays.

One to one activities include: Reminiscing, Music & Memory (sing-a-longs), Adult coloring, modified card games.

Volunteers (of all ages) bring the community into Our Home. Once a volunteer application is completed to indicate the individual’s availability, (and a clear Criminal Reference Check is submitted), an activity schedule is created and posted for Residents to view.

During the Pandemic, virtual visits were scheduled with family members unable to travel or come into Our Home. These video calls were scheduled seven days a week.

Food Services

We have a registered dietician, nutritous meals and snacks, monthly special event meal activities.
