OVS REDUCED SERVICES UPDATE

Amid the MCA non-essential service closure, the Office of Vital Statistics has reduced its services temporarily.
 
As of November 2, they will be issuing laminated status cards only; priority will be given to those who have lost or damaged cards. Cards will be issued by curb-side service and all efforts will be made to complete the application virtually.
 
OVS is open Tuesdays & Thursdays by appointment only. Those individuals who have already scheduled appointments for this week should attend our offices at the agreed-upon time and date.
 
When obtaining a status card you will be expected to:
1) Whenever possible, provide a photo by email to ovs.office@akwesasne.ca or text message sent to 613-361-9144 prior to your appointment. Include the name of the individual and date of appointment with the photo being sent. Photos will be taken on date of appointment if you cannot provide a photo in advance.
2) Wear a mask.
3) Complete a COVID-19 screening twice: The first will be conducted by telephone prior to an appointment being made and the second will be conducted in-person at the time of your appointment to obtain a status card.
4) Call to reschedule your appointment if you exhibit any symptoms on the date of your appointment.
5) Be prepared to show identification.
 
Criteria for photos:
• No headgear or hoods
• No filters
• No sunglasses
• Recent full-face photo (including shoulders)
